The Oral & Facial Surgery Center is open Monday – Friday from 8:00am until 5:00pm. We will schedule your appointment as promptly as possible. Because we value your time and want to make your visit with us as smooth as possible, we need certain information from you:

Having your patient registration and health history filled out in advance is greatly appreciated. For your convenience we have the New Patient Paperwork Form online. Please fill out and bring this to your appointment.
We also request that you bring all insurance information with you. This will ensure that you receive the maximum benefit from your coverage, and allow timely and accurate claims submission to your carriers.
If you received a referral slip and/or x-ray(s) from your dentist or physician, please bring it with you to help us optimize your care.

We try our best to stay on schedule to minimize your wait time. Due to the fact that Dr. Tolley provides surgical services, various circumstances may lengthen the time allocated for a procedure. Emergency cases can also arise and cause delays. We appreciate your understanding and your patience.
