Your First Visit


Patient shaking hands and introducing to hygienist at Oral and Facial Surgery Center Your initial appointment with us will consist of a consultation explaining your diagnosis and treatment options. Occasionally, surgery can be performed the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.

"Our office prides itself in detailed pre-operative consultations so that we can deliver the very best clinical care and anticipate surgical outcomes".

Please assist us by providing the following information at the time of your consultation:


•  Complete your Patient Registration Forms. If printed online, please bring them with you to your appointment. You may also fill them out in our office but please arrive at least 20 minutes early.
•  Please bring your referral slip with you or have it sent to our office in advance. The referring office can email, fax, or mail your referral slip to our office. This referral is of high importance as its shows Dr. Tolley and our surgical staff what your general dentist or physician would like to have evaluated.
•  Please bring your current x-ray(s) with you to your appointment. You can also have the referring office email or mail a copy to us. If you don't have a current x-ray (within a year), we can take one at our office but additional cost could be accrued.
•  Please bring your dental and medical insurance cards (both) to your appointment. If this information is not available, payment is due in full at time of service.
•  A list of all medications and dosages.

IMPORTANT: All patients under the age of 18 must be accompanied by a parent or legal guardian at the consultation visit.

Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e., diabetes, high blood pressure, artificial heart valves, rheumatic fever, etc.) or if you are currently taking any medications (i.e., heart medications, aspirin, anticoagulant therapy, etc.).

Cancellation Policy and Rescheduling:


If you need to cancel or change your appointment please call our office at least 24 hours in advance of your appointment time. Additionally, if you have scheduled your surgery and need to cancel or reschedule, we ask that you give us at least a 72 business hour notification to avoid our broken appointment fee of $250.00. We do need verbal confirmation for any changes. Office hours are Monday - Friday from 8:00am until 5:00pm and you may reach us at (850) 386-4602.
