New Patients are always welcome. We look forward to serving you!

If you need to cancel or change your appointment please call our office at least 24 hours in advance of your appointment time. Additionally, if you have scheduled your surgery and need to cancel or reschedule, we ask that you give us at least a 72 business hour notification to avoid our broken appointment fee of $250.00.

We do need verbal confirmation for any changes. Office hours are Monday – Friday from 8:00am until 5:00pm and you may reach us at (850) 738-1349.
